When you come for your appointment please remember to obtain and bring the following
• Drivers License or a valid ID
• Insurance information
• Referral Letter (if required)
• Copies of results, x-rays, MRI’s. CT scans etc and any other relevant information
• List of medications (if any)

What is your cancellation policy?
If you need to cancel an appointment

We recognize that your time is valuable, and we make every effort to run on time. Occasionally emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologize if we keep you waiting.
What facilities are available on-site?
Dr. Wein’s clinic offers:
casts, splints, and braces
orthotics, pedorthotics and prosthetics
Do you examine and treat children?
Yes. Dr. Wein is a father of two, and is good at helping children feel comfortable when getting treatment.
Will I need anesthetic?
It depends on the procedure. The anesthetics we use vary from localized numbing agents to general anesthesia. We’ll consult you about everything as we go through the process.
What is your parking situation?
All of the clinics that Dr. Wein attends have free, spacious parking lots. The hospital where Dr. Wein does surgery has ample space in several parking decks.
